The Neuro Practice Carer Support provides personal care and home help services to support you to live independently in your home and in your community. This guide outlines your rights and responsibilities as a participant and describes our commitment to providing a quality service to you.
The Neuro Practice Carer Support has been created to assist participants who require therapy, support or personal care assistance. Carer Support provides personal care, community access support and respite care for people with all levels of need. Personal care attendants help a wide variety of participants including people who are disabled, chronically ill or have difficulty with their ability to plan and carry out daily activities.
In addition to the attendant care team we have access to Physiotherapy team members who can assist with training when required to work with you to achieve your daily goals and promote assisted independence.

Participant and team member inclusion
In Home Support
The Neuro Practice Carer Support provides assistance with day to day tasks and activities, so you can remain living at home.
Providing quality support begins with planning and assessment.
Before starting new services, we complete a support plan with you to make sure that:
We understand and discuss your preferences and choices about what, how and when services are provided to best suit you
We are aware of any specific health conditions that may impact on your support and your team members providing support to you
Your home and other environments where our team members support you are safe for both you and the team members.
Our individualised support plans are reviewed and evaluated at regular intervals.
Personal Support services include a range of activities such as:
Hygiene, grooming and dressing
Support to complete therapy and exercise programs including hydrotherapy
Meal preparation and assistance with eating and drinking
Help to utilise aids and equipment such as hoists and wheelchairs
Community outings and appointments, leisure and vocational activities.
Please Note: Domestic support/cleaning services can be provided as part of a package or separately to personal care services.
The Neuro Practice Carer Support has a focus on ensuring its participants have every opportunity to be active and valuable members of their community.
Community inclusion for participants has included;
Accompanied 1 : 1 trips interstate
Transport/assistance to see events, shows and exhibitions
Travel to AFL games
After school activities
Support to attend educational seminars and groups
Opportunities to attend social gatherings
Assistance to attend appointments and therapy programs
We are able to assist you with your therapy both at home and in the community to promote improved independence including:
Home exercise programs to increase strength, balance and mobility
Gym/strengthening programs tailored to your level of physical ability
Assistance with activities of daily living including meal preparation, personal care, weekly planning and domestic tasks
Help with use of communication aids and designed speech therapy exercises
Attendance at peer and social groups to help you feel confident as part of the community

Director/ Care Manager
Jodie, together with Rachael, launched the Neuro Practice Carer Support as a way of offering a more specialized service to participants in the disability sector. After an accident impacted a member of Jodie’s family in 2011, which led her family into navigating through the health system, there appeared to be a very real need for families to have access to an attendant care agency that focused on the participant and their needs.
The Neuro Practice Carer Support has applied a Holistic approach to it’s participants and with their families build care program’s based on individual needs.
